Whether you're coming in through your auto insurance, Medicare, workers comp, or you’re not using insurance at all, this page outlines what to bring, what to expect, and how your care will be covered.

General Insurance

Before You Arrive:

  • After scheduling, you’ll receive an email with a link to set up your secure online patient portal, where you can complete intake forms and manage your account.

  • We’ll verify your insurance benefits in advance.

  • If you haven’t submitted your forms online, we’ll ask you to fill them out in-office before your appointment.

Bring With You:

  • Your insurance card (we’ll keep a copy on file)

  • Photo ID

  • Any completed paperwork

  • Referral form, if referred by another provider

What to Expect:

  • On arrival, you’ll review and sign the NWSS Insurance Form, which confirms your verified benefits.

  • Our front desk team will walk you through any questions and assist with any pre-authorizations, if required.

Coverage Breakdown:

  • Most plans cover chiropractic and therapeutic care depending on your policy.

  • Any non-covered treatments or cost-sharing (like copays or deductibles) will be discussed with you in advance.

Auto Injury

Before You Arrive:

  • After scheduling, you’ll receive an email with a link to set up your secure online patient portal, where you can complete intake forms and manage your account.

  • Make sure a medical claim has been opened with your auto insurance provider (or the provider of the vehicle you were in).

  • Complete your Auto Injury Form, including:

    • Adjuster or claim information

    • Date of Incident (DOI)

    • Your current health insurance (used when PIP funds run out)

Bring With You:

  • Insurance card(s)

  • Photo ID

  • Auto Injury Form details

What to Expect:

  • Our team will verify your PIP coverage and discuss how it applies to your treatment.

  • You’ll be responsible for tracking your own use of PIP funds.

Coverage Breakdown:

  • Chiropractic and therapy care are typically covered through PIP (Personal Injury Protection) for up to one year from the Date of Incident.

  • Once PIP is exhausted, coverage will transition to your health insurance or become your responsibility.

Medicare

Before You Arrive:

  • After scheduling, you’ll receive an email with a link to set up your secure online patient portal, where you can complete intake forms and manage your account.

  • No pre-authorization is required for your initial visit.

  • If Medicare requires pre-authorization later, you will be asked to sign an ABN (Advanced Beneficiary Notice) before receiving care.

Bring With You:

  • Medicare card

  • Photo ID

Coverage Breakdown:

  • Covers “medically necessary” spinal chiropractic manipulation at 80%, after your deductible is met.

  • You are responsible for the remaining 20% unless you have secondary insurance.

  • Exams and therapy treatments are not covered by Medicare.

Workers Comp

Before You Arrive:

  • After scheduling, you’ll receive an email with a link to set up your secure online patient portal, where you can complete intake forms and manage your account.

  • Request an 801 Form from your employer and complete it prior to your visit.

  • If this is your initial claim or you’re switching providers, you’ll also need to complete an 827 Form when you arrive.

Bring With You:

  • Workers Comp claim details

  • Completed 801 Form

  • Photo ID

Coverage Breakdown:

  • Coverage includes 100% of care for 60 days or 18 visits, whichever comes first.

  • If additional treatment is needed, a new referral from your primary care physician is required.

Chiro Health USA (CHUSA)

What to Expect:

  • You’ll sign the CHUSA Form upon enrollment. You may add dependents for free.

  • If you're opting out of insurance, you’ll also sign an opt-out form, which legally allows NWSS not to bill your insurance provider.

Coverage Breakdown:

  • CHUSA provides a discounted, time-of-service rate for all treatments.

  • Payment is due at time of service to receive the discounted rate.

  • Visit chirohealthusa.com for details or ask us during your visit.

Before You Arrive:

  • After scheduling, you’ll receive an email with a link to set up your secure online patient portal, where you can complete intake forms and manage your account.

  • Ask our front desk team about signing up if you're uninsured or your insurance isn’t contracted with us.

Bring With You:

  • Photo ID

  • CHUSA Form, if already enrolled

No Insurance / Cash Pay

What to Expect:

  • We’ll walk you through estimated costs before your visit.

Coverage Breakdown:

  • All visits are patient responsibility at 100% of fees

  • If we’re not contracted with your insurance or your provider doesn’t offer sufficient coverage, we offer a discounted option through Chiro Health USA. Ask us how to enroll at your next visit—our front desk team will be happy to help.

  • No forms required unless you’re opting into Chiro Health USA.
